Darling Downs and West Moreton PHN is one of 31 Primary Health Networks across Australia funded by the Commonwealth Department of Health to improve primary health care (health care delivered outside of a hospital setting).
Our key objectives are to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
Our PHN achieves this by working with local health care stakeholders and community to fund programs and services based on local health needs and support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients.
We also support the development of a local sustainable health care workforce.

Services Offered
Darling Downs and West Moreton PHN funds a number of services and supports to assist people with mental health, suicide prevention and alcohol and other drugs.
We work with local communities, consumers, carers, health professionals, hospitals and community health providers to design local solutions to meet local needs. Our PHN funds mental health services with a person centred stepped care approach.
Stepped care offers a flexible hierarchy of interventions, from the least (low intensity services for people at risk of or experiencing mild mental health conditions) to the most intensive (high intensity services for people with severe and persistent mental health conditions), matched to a person's need. Stepped care ensures that people can access the most appropriate service at any given time and move through the continuum of care as their needs change.
You can self refer to some services, while a GP referral is needed for others.
We can help people of any age by providing counselling, specialised allied health therapies, culturally appropriate mental health services and general support.

A community counseling agency is a group therapy practice that offers affordable mental health services. Most are independent non-profit agencies, state agencies, or publicly-funded agencies. A few may restrict who is eligible for services, so it is a good idea to check when you call.
Community counseling agencies are generally more affordable than other therapy providers, but how much more affordable they are will depend on whether you qualify for a discount or sliding scale fee (or if they accept your insurance). Many, but not all, have a policy that they won't turn away anyone due to inability to pay. You should ask about their fees when you first call.
Expect to talk to kind people who want to help you find the care you need. Most community agencies strive to connect you with a live person within 24 hours, if not immediately. If you're asked to hold or leave a message, don't give up; just leave a message and wait. You should hear back pretty quickly.
Most agencies try to set up an initial assessment appointment within a week (some do within 24 hours), though the waitlist to start therapy is usually longer—about a few weeks on average. If you're not eligible or if the agency is not right for you, it's usually still worth it to call or drop in, because staff are knowledgeable about local options and can often refer you to one.
